A Dangerous Occurrence is an unplanned and undesired occurrence which has the potential to cause injury, and which may or may not cause damage to property, equipment or the environment. Accidents and Dangerous Occurrences may result from a sequence of events and circumstances involving a combination of unsafe acts, unsafe conditions, system failures, human factors and/or omissions. The ҹ޸þ is required under law, to report promptly, any occupational accident which results in any worker being injured and being unable to perform his/her normal duties, for more than 3 consecutive days to the Health and Safety Authority (HSA). (Even if those days normally included a weekend/ scheduled rest period). Certain prescribed dangerous occurrences must also be reported to the HSA. Accidents and Dangerous Occurrences which must be notified to the Health and Safety Authority (HSA) are set out in the Safety, Health and Welfare at Work (General Application) Regulations current edition. Any fatal accidents must be reported straight away by phone or e-mail to the HSA. In addition, the staff elected Safety Representatives have a statutory right to investigate accidents and dangerous occurrences involving UCC staff. Accordingly, the Safety Representatives and the ҹ޸þ Health and Safety Office and B&E/ Facilities and OCLA insurance need to be advised of such incidents, as soon as they occur. The ҹ޸þ is also required, under the terms of our insurance policies, to report all accidents, fires and other occurrences to the College insurers, to facilitate prompt and efficient claims handling and response to any employers or public liability claims lodged via the national PIAB system. An even more important dimension to the collection of information on accidents and dangerous occurrences, is the requirement of facilitating departments and the ҹ޸þ to collectively learn from each accident/incident so as to prevent re-occurrence. This is to be achieved by School/Department management promptly conducting an investigation. Note: Personal medical incidents not involving an injury are not an occupational accident. Accidents incurred on personal time outside of work are not treated as UCC work related accidents i.e. travelling to and from UCC/a normal place of work. Sports injuries are not considered occupational accidents. (These are to be notified to the Department of Sport and Physical Activity where they relate to a UCC activity or sports facility.) OCLA insurance policies may also assist with minor costs. 10.2 UCC INTERNAL REPORTING SYSTEM As of June 2025, incident reports should be submitted via the Ivanti Helpdesk. Any person may report an accident or a dangerous occurrence. Investigations of incidents should be submitted via MS Forms, but the investigation form will also migrate to the Ivanti Helpdesk in due course. The webpage, linked below, will direct users to the relevant forms. All incidents, dangerous occurrences and near misses are reportable via one form, accessible to UCC staff members and students. Should members of the public, guests or contractors need to report an incident, they should contact the ERM Office at HYPERLINK "mailto:erm@ucc.ie"erm@ucc.ie. In the case of serious incidents an investigation report form must be completed in a timely manner by the relevant College/School/Department/ Centre/Unit Head or their nominee.
