The ҹ޸þ permit to work system is a formal written procedure used to control (a) the activities of all contractors undertaking work of a physical nature (with fire \ safety risk potential) and (b) College \ School \ Department \ Centre \ Unit staff engaged in certain type of works i.e. work which is identified as hazardous (high risk) and where special precautions are necessary to control these hazards. It should be used for all, contractor activities and for ҹ޸þ employees, who may be involved in hazardous high risk work situations (whether routine or non-routine or primarily of a non-routine nature). It is essentially a document which sets out to identify the hazards and implications for staff \ students and ҹ޸þ activities of the work to be done and the precautions to be taken and includes for an imbedded risk assessment of the tasks and controls and necessary internal consultations. It seeks to promote a proper assessment of the risks and adaptations of appropriate safety procedures before work commences in a pre- agreed manner and at pre-agreed locations and time frames. It is a clear record that all foreseeable hazards have been considered in advance and that all appropriate precautions are defined and taken in the correct sequence. It does not, in itself, make the job safe, as it is dependent for its effectiveness on the competence \ experience of people concerned to carry out the work as specified \ proposed and indeed appropriate levels of monitoring and supervision commensurate with the residual risks. The above system is required for: all work undertaken by contractors undertaken work of a physical nature on behalf of Colleges/Schools/ departments, both building and maintenance or diagnostic/ survey related and, all high risk work undertaken by Buildings Office direct staff/labour whether routine or non-routine) and all high risk work undertaken by Colleges \ Schools \ Departments \ Centres \ Units, where such hazardous work is carried out and controlled by the local management (duty holders). This is to protect UCC and those involved and others who may be impacted and the environment. The permit to work systems will require to be operated by the Buildings Office/Facilities and General Services primarily and additionally in other Colleges \ Schools \ Departments \ Centres \ Units such as the Computer Centre; General Services; Audio Visual Services; Tyndall, Engineering \ Science Departments, IT and Library Services who engage contractors for physical works (incl maintenance). Permits shall be implemented in conjunction with the relevant College \ School \ Department \ Centre \ Unit, as per the requirements of Section 14 of this safety statement. Strict control of work carried out by non-departmental personnel is necessary to avoid unnecessary related accidents. The permit to work form specifically provides for the person in charge of the area to hand over control of the area, before work commences and to identify special hazards/precautions required. A protocol for accepting hand back of the area is also included. Training in the permit to work system shall be provided by the engaging UCC College \ School \ Department \ Centre \ Unit to all contractors and relevant employees. 16.2 SAFE OPERATING PROCEDURES ( safe systems of work) (*) Many injuries and losses may occur through poor work practices. This can only be be achieved if each activitie is Risk assessed by College/School/Department management, appropriate controls are identified and applied. This to include every employee following safe systems or work (the Safe Operating Procedures) the arrangements for safe working - primarily those of a routine nature or specific tasks executed regularly by competent experienced staff ( variable risks). It is recommended that each College \ School \ Department \ Centre \ Unit prepare safe operating procedures for all routine \ non-routine activities within their respective areas, irrespective of risk level, which entail the use of mechanical equipment, chemical\ biological \ physical agents or high risk maintenance/repair activities. It is the responsibility of each College \ School \ Department \ Centre \ Unit Head that they are revised and updated to take account of experience and new activities as they are introduced and to effectively train staff as to their content and limitations. SOPs are of themselves are limited in scope and focus and may not be universally relied upon as complete risk assessments for activities defined within a very narrow range of parameters in a fixed environment and of a low risk with little variation in work, personnel or environment or where there are multiple competing risks and trades/ contractors involved in an activity that require co-ordination. College/School/Department management must conduct and record adequate risk assessments, apply the appropriate hierarchy of risk control and apply the Principles of Prevention prescribed in the SHWW Act (current edition) within or in addition to any SOPs. ( In most cases that will necessitate Risk assessments being discharged/ undertaken separate to SOPs as necessary). The safe operating procedures will none the less play an important role in controlling and regulating uniformity in systems of work and reducing hazards and risks. They will be an essential element in the training of new and existing employees on safe systems of work in fixed work at fixed environments such as laboratories and in laboratory testing /procedures. The College \ School \ Department \ Centre \ Unit Head or his designate is responsible for preparing and propagating same and all associated staff information and training so as to apply the provisions of SOPs effectively. Following training it is incumbent on each employee to carefully read and understand the safe operating procedure sheets and acknowledge receipt by signing on the appropriate register, which College/School/Department management are encouraged to put in place and maintain. ( record of training and dissemination of information). 16.3 SPECIMEN SCHOOL/DEPARTMENT PERMIT TO WORK FORM ( Template) A revised electronic template for a School/Department General Permit to Work form for adaptation and use by schools/departments within ҹ޸þ is available on the Health and Safety Office Web page and within the Buildings Office ( joint collaboration). As well as the General PTW it contains specifically customised permit to work form is required for certain categories of hazardous work (*), e.g. Hot Work, Confined Spaces Entry, Dangerous Machinery, Electrical Work, Cold Work x and Flammable Atmospheres. x Some categories of Cold Work, such as use of radioactive isotopes, are more hazardous than above examples. In these cases, a higher level of authorisation and experience is necessary. These requirements should be clearly stated on the permit form. NOTE: Items marked with an asterisk (*) apply primarily to Applied Scientific Departments, the Buildings Office and General Services (window cleaning operation). To summarise the UCC required systems is as follows A PTW IS REQURIED FOR ALL CONTRACTORS UNDERTAKING PHYSICAL WORK AT UCC/ ON UCC PREMISES. This responsibility rests with the management of the engaging UCC Functional Area/College/School/ Department, Centre or Unit. The PTW is a means of regulating their entry and presence in a pre-agreed fashion/area to execute an agreed tasks in an area which is legally controlled by UCC ( not legally transferred to the contractor). The movement of MEWPS and specified categories of large vehicles and trailered vehicles on any / all UCC property/ UCC controlled sites is to be strictly controlled by a Permit to Work. This PTW should only be signed off when the contractor fills in sufficient detail on our permitform to identify to us the risks they are bringing to UCC and only signed off/ UCC approved when we are happy with the UCC relevant impacts of the when and how of the job and we buy in to the timing and method of access and impacts on our premises and operations and people. A PTW IS RQUIRED FOR HIGH RISK WORK ACTIVITIES INVOLVING UCC STAFF PTW issued for hazardous/high risk works undertaken directly by UCC staff , (whether routine or non-routine work) is also a long standing H&S requirement of the UCC Dept. Safety Statement work manual and long standing liability loss control mechanism in industry - particularly process industries. All routine and non-routine high risk work undertaken by College \ School \ Department \ Centre \ Units or by Buildings and Estates direct staff/labour and /, shall be controlled by a permit to work issued and controlled by the relevant management ( duty holders for the work of their staff) following RA and application of appropriate risk controls to mitigate the risk and or reduced the exposed population. A PTW IS RQUIRED FOR the movement of categories of large vehicles, MEWPS, trailered vehicles and large construction vehicles and plant including associated deliveries.
